Filing a Claim
Once you press "Submit Claim", your claim will be filed, you will receive an email confirming your claim, and you will be automatically re-directed to your personalized claim page. On this claim page, you can check the status of your claim, correspond with our claim agents, and upload any files that have been requested.
Please make sure your information is correct. The more information you provide, the better our agents can assist you in the claims process. Any false or inaccurate information may result in your claim being closed automatically.
By clicking "Submit Claim", you certify that all information on this claim is accurate and truthful under Federal Law and under penalty of perjury, under the laws of the state of California.
The submission of a false, fictitious or fraudulent statement may result in imprisonment of up to 5 years and a fine of up to $10,000 ( 18 USC 1001 ).
In addition, a civil penalty of up to $5,000, and an assessment of twice the amount falsely claimed may be imposed ( 31 USC 3802 ).
